This project 'Transition: understanding it and making it work' looked at how young people with long term health conditions are transitioned from one service to another. The study aimed to find out more about transition from the point of view of young people, parents and health professionals - to discover what works well, what does not work well and what could be done to improve services for young people.

These benchmarks for transition have been developed with young people, parents and professionals with the aim of improving transitional care for young people and enabling the sharing of best practice between services. As such they offer an example of a ‘practice guide’ to support transitional care.
